You are invited to attend our first Community Support Network (CSN) advisory meeting on March 24, 2025, at the Kauffman Foundation Conference Center. This focus of this session will be “Strengthening Clinical and Community Links in the Kansas City Bistate Area.” The meeting will be convened by the Mid-America Regional Council (MARC), KC Health Collaborative, HealthTeamWorks, and the Center for Practical Bioethics, in partnership with USAging’s Center of Excellence to Align Social Care.
The CSN would like to thank you for your letters of support, robust discussions and interest in working together to strengthen the community care hub model[VJ1] in and around the Kansas City region. While the CSN is not new to Kansas City, and was instrumental during the pandemic, we now have an opportunity to further refine and expand the ideal model for this region.
This initial meeting will bring together leaders, advocates and experts for an overview and discussion of the community care hub model in our region. The CSN Advisory group will inform this approach, collectively impacting health, engagement, utilization and cost metrics, while strengthening resilient community resources.

Ongoing efforts of the CSN Advisory will focus on augmenting clinical / community links in the pursuit of a seamless care continuum and better outcomes. Initial topics may include:
Identifying shared goals and objectives and building a robust community network that meets people where they are.
Identifying and problem-solving barriers to meet the needs of individuals with complex chronic care needs and challenges that impact outcomes and cost.
Identifying priority populations, capacity gaps in home and community-based services (HCBS), and barriers to accessing care within the CSN area.
Partnering on measures of success in this model and learning from best practices in other states.
We look forward to collaborating with you to strengthen the region’s healthcare landscape, community infrastructure and shared impact.
